Personal Injury Introductory Information

Thank you for trusting us in this vulnerable time. Personal Injury is a situation where you have been injured in an auto accident and are seeking care. This will be covered either by your insurance, the person at fault's insurance, or through an attorney. Either way your health is what we are most concerned about. There are a few things that we need to prepare for your recovery. If going through auto insurance for coverage, we ask that you collect the following information prior to your visit with us.

-Insurance Provider of the Payer

-Claim Number

-Person at Fault's Name and DOB

If sent from an attorney, we will handle everything directly with them. From here we will focus on getting you back on track. You will not be directly billed for anything so long as there is coverage through the options above.

Here’s What to Expect:

Visit 1: Consultation + Examination-> x-rays

We want to hear you tell us all about you, and we want to learn more about you to gather some baseline work. We will send you to our partners of Meridian Radiology so they can take a few pictures of your spine (your insurance will cover these costs if applicable).

Visit 2: Report of Findings + A𝒅𝒋𝒖𝒔𝒕𝒎𝒆𝒏𝒕𝒔 + T𝒉𝒆𝒓𝒂𝒑𝒊𝒆𝒔

Because Knowledge is power, we will explain your x-rays, go through a plan of action, and make everything make sense. Your first adjustment will take place along with therapies to make your healing process well-rounded and complete.
